As the parent(s) or legal guardian of the above child, I/we authorize any adult acting on behalf of Gan Shalom Preschool & Day Camp (GS) to hospitalize or secure treatment for my/our child.  It is understood that if time and circumstances reasonably permit, GS personel will try, but are not required, to communicate with me/us prior to such treatment.

By signing this waiver and release, I hereby acknowledge and agree that I, on my behalf and on behalf of my child(ren): a) understand the contagious nature of COVID-19; b) voluntarily assume the risk that me, my child(ren) or anyone for whom I am responsible may become exposed, infected, or otherwise contract COVID-19 while attending, participating in or, otherwise engaging in any activities at or in connection with GS and Chabad on the Avenue. I hereby waive, release and discharge Chabad on the Avenue and its subsidiaries, their employees, agents, operators, directors, officers, volunteers, and independent contractors (“Releasees”) from any liability, claims, damage or injuries arising from exposure to, or contracting, COVID-19 while at GS. I AM AWARE THAT BY SUBMITTING THIS FORM I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE, WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS AND ASSIGNS MAY HAVE AGAINST THE RELEASEES.

Upon registering your child, a non-refundable deposit will be charged to your credit card. This deposit goes toward tuition. Once your child is accepted into the program we require post dated cheques mailed or dropped off or you can opt to pay by credit card which will be charged as per the payment plan (3% credit card processing fee applies.) Please see the Tuition Schedule for the payment plan. Please note that there are no tuition credits or make up days for illness, holidays or family vacations. We require a one-month notice in writing if it becomes necessary to withdraw your child from the preschool. At that point your remaining post-dated cheques will be returned to you or we will cease to charge your credit card.
